HOME & COMMUNITY CARE CO-ORDINATOR (NURSING) PERMANENT

22.5 TO 30 HOURS PER WEEK
(TO BE NEGOTIATED)

 

SUMMARY

The priorities of the Home & Community Care Coordinator will ensure that the needs of eligible clients are being met in a timely and appropriate mechanism of service delivery, through coordination and advocacy of all available services. The main role is to encourage community members to contribute to improve community health conditions, and assume an active responsibility for improving and maintaining their individual level of health. This position requires the provision of home care nursing services. The Home and Community Care Coordinator will report directly to the Manager of Health Programs.

Specific to this position, the Home & Community Care Coordinator will oversee and support the Early Childhood Development program and Healthy Babies/Healthy Children program. The Home and Community Care Coordinator will be responsible for assisting community members, infants to the elderly, in achieving an optimum level of health. Also, the Home & Community Care Coordinator will maintain a good working relationship with all clients, governments, agencies, native organizations, associations and business associates.

DUTIES:

* Ensure compliance with all applicable legislation and Council policy and procedures

* To act as interpreter where necessary between members of the community and health care delivery team. Assist visiting officials in the conduct of their business to First Nation people.

* On-going liaison with all off-reserve home care service providers, such as the Community Care Access Centre; Victorian Order of Nurses; General Practitioners and Hospitals; Nursing Homes; Long Term Care Regional Branch Office; Public Health Unit

* Administer the Home and Community Care, Healthy Babies/Healthy Children, and Early Childhood Development Program(s), budget's, reports, etc.

* The Coordinator will also demonstrate a working knowledge to fulfill the components of the Healthy Babies/Healthy Children requirements i.e. completing assessments, etc.

* Assist in the selection, evaluation and professional development of the Personal Support Worker(s).

* Increase awareness of programs and services to clients, community health care providers and all community residences.

* Ensure that all required documentation regarding the delivery of care and services is taking place within the client Home & Community Care Program charts.

RESPONSIBILITIES:

* Acts as case manager for all clients to ensure timely, quality, compassionate care in all required areas of service

* Ensure participation of client and families in setting goals of service

* Ensure that all caregivers adhere to safe practices in all areas of service delivery

* Conduct business in a manner that reflects positively on the program reviews and recommends all services, supplies and equipment necessary to achieve program goals within the approved budget

QUALIFICATIONS:

* Registered Nursing diploma required with two years demonstrated experience preferred.

* Current membership, in good standing, with the College of Nurses of Ontario.

* Valid Certification with: CPR & First Aid; Driver's Licence

* Excellent organizational, interpersonal, written and oral communication skills.

* Case Management experience an asset.

* Excellent group facilitation skills.

* Knowledge of the history, dynamics and culture of the Alderville First Nation community and First Nations in general is preferred.

* Ability to network effectively and productively with related organizations and agencies both within and outside the community

* A demonstrated knowledge of computer experience i.e. Microsoft Office, Corel, Internet

SALARY RANGE:
To commensurate with experience and qualifications.
